Weekly Status Reports

 

Status Reports and Timesheets are required for client billing and for your payroll to be processed.

Your status report will need to include your name, the dates the report covers, what your accomplishments were during that period, your goals for the next period, and any issues or problems.

Once filled out you can e-mail the status report to status@appliedperformance.com or you can fax it into our office at (614)847-9599.
